Advanced Home Care and Hospice of Northern Idaho is looking for an RN case manager to join our growing home health team in Lewiston! Candidates will be friendly, positive and focused on excellence! Job Responsibilities:

• Perform admission, discharge, and routine home health visits.
• Coordinate & communicate patient care plans with interdisciplinary team.
• Document all phone calls and visits using our EMR.
